To verify if a provider is in the network, please call (888) 357-6767 or check their website, www.humana.com Please take the time to verify that your doctors, hospitals, and/or facilities are in the network prior to making any plan changes. (Note: The Trust cannot guarantee that a doctor, hospital, and/or facility will remain in the network.

Information from the Insurance Office 2/10/09:
Humana has only one claim form. This form can be used for medical and vision and can be downloaded on the link below. Also below is a link to download a provider nomination form. The provider nomination form must be filled out by the doctor or the member (must include doctors Tax id or it will not be accepted) and returned to our office.
Attention members: If you have any questions to contact the insurance office directly so we can help them. Sometimes a question is asked on your website and the wrong answer is given. Thank you for your assistance.
